Age of applicant should be minimum 18 (eighteen) and maximum 30 (thirty) years as on 24/08/2013 AD. However, the maximum age limit is 32 (thirty-two) years in case of sons and daughters of brave freedom fighters and sons and daughters of freedom fighters and physically challenged. Affidavit is not admissible for proof of age. Age will be proved on the basis of date of birth given in SSC Certificate/National Identity Card/Birth Registration Certificate.
Candidates working in Govt/Semi-Government/Autonomous/Semi-Autonomous Organizations must apply within the stipulated time after taking the approval of the appropriate authority as per the Government Rules (within the prescribed age limit) and no objection letter issued by the appointing authority at the time of oral examination of all the post candidates ( Original copy of NOC) must be submitted compulsorily.
If the permanent address mentioned in the application form by the candidate is different from the permanent address mentioned earlier in any certificate or elsewhere or if the address of the husband is used in case of female candidates, then in that case the candidate should submit the National Identity Card (NID) of the concerned City Corporation in favor of the changed permanent address. Citizenship certificate signed by Mayor/Ward Commissioner/Municipal Mayor/Councillor/Union Parishad Chairman should be submitted. 6. For the post of Health Assistant, the candidate must be a permanent resident of the Ward (Old) for which he/she is applying. Permanent residents of other wards of the same union can also apply. However, if the eligible candidates are not available from the residents of the respective ward, the eligible candidates of the neighboring wards will be selected.

The list of Health Assistant vacancies (Ward, Union, Upazila wise) will be available on the notice board and website of the Office of the Civil Surgeon and the Office of the Upazila Health and Family Planning Officer.
If any information provided by the candidate is proved to be secret, untrue or false or if any kind of fraud or corruption is resorted to before or after the examination, even after the appointment, legal action will be taken against the candidate including canceling the candidature or appointment of the candidate.
If a candidate is not a citizen of Bangladesh or has married or is committed to marry a person who is not a citizen of Bangladesh or has been convicted by a criminal court on charges of moral turpitude or has been dismissed from the service of any government or autonomous institution or local authority, he/she may apply. Do not qualify for
Candidates can apply for only one post. If applying against more than one post, all his applications will be considered cancelled.

The application deadline and terms/conditions are as follows:

(a) Online application form filling and examination fee submission start date and time 24/08/2023 AD from 10.00 am
(b) Last date and time for submission of application online is 14/09/2023 AD upto 05.00 PM.
(c) Candidates who have received the User ID within the said period can submit the examination fee through SMS within the next 72 (seventy two) hours from the time of submitting the application online.

Candidates have to scan their color photograph (length 300 × width 300 pixels) and signature (length 300 × width 80 pixels) in the online application form and upload it at the designated place. Image size should be maximum 100 kb and signature size should be maximum 60 kb.
Since the information filled in the online application form will be used in all subsequent activities, the candidate himself will be hundred percent sure about the correctness of all the information filled before submitting the online application form.
Candidates shall keep a color print copy of the online filled application form as a reference for any examination related requirements.

Rules for Sending SMS and Payment of Test Fee:-

After completing the online application form by properly filling the application form and uploading the photo and signature according to the instructions, the application preview with the photo can be seen on the computer. Candidates who have submitted the application form correctly will get an Applicant’s Copy with a User ID, Photo and Signature.

Candidates should print or download the Applicant’s Copy and save it. A User ID number will be given in the Applicant’s Copy and later using the User ID the candidate can send 02 (two) SMS through any Teletalk pre-paid mobile number regarding the examination fee for the post Serial No. 01 to 03 (Tk. 300/-) and teletalk service charge of Tk.34/-) total Tk.334/-(three hundred and thirty four) offerable, for serial no.04 to 09 post (Tk.200/- including service charge of Tk.23/-) total Tk.223/- (two hundred Twenty-three) Tk non-refundable and for serial no-10 post (including Tk 100/- and teletalk service charge Tk 12/-) a total of Tk 112/-(one hundred times) Tk non-refundable within 72 (seventy two) hours.

(d) Character certificate issued by a gazetted officer of the first class
(o) 03 (three) copies of attested passport size photograph of the applicant.
(f) Freedom fighter certificate/certificate issued by Ministry of Liberation War Affairs as per the latest policy of Govt.

(g) Certificate issued by the concerned City Corporation/Municipality Mayor/Union Parishad Chairman in case the applicant is the son-daughter or son-daughter of a freedom fighter or son-daughter of a freedom fighter.
(h) Certificate issued by the concerned District Commissioner in case of minority candidates.

(j) In case of orphans and physically challenged candidates, certificate issued by the Deputy Director of District Social Services Office/ concerned officer in charge.
(j) Certificate issued by District Commandant/District Ansar Adjutant in case of Ansar and Village Defense candidates following Government Rules. (k) Certificate as proof of computer proficiency where applicable.
(l) Color print copy of downloaded Applicant’s Copy and Admit Card.

In the light of the recruitment circular issued on the basis of Health Department’s Memorandum No.Sw.A.D./Pr.-2/3rd Class Recruitment-4/2018/6354 dated: 11/12/2018 AD to the candidates who have previously applied online for the posts of Statistician, Health Assistant and Laboratory Attendant. No need to reapply. Their application will be considered admissible if correct and their age will be reckonable as per the conditions of previous recruitment circular. They can now participate in the said recruitment test based on the recruitment circular issued on 20/08/2023 AD and eligible candidates will be issued admit card to participate in the said recruitment test. 24 No daily or traveling allowance will be given to the candidate for appearing in the written, practical (where applicable) and oral examinations.
